There is an exciting opportunity for a qualified nurse with an experience of working with children and young people with mental health and/or a learning disability available to join the Children's Continuing Care team, as a Senior Case Manager. The role entails working 37.5 hours per week on a permanent basis with the team based in Middlesbrough
The children's continuing care team consists of over 15 staff and are part of the All Age Continuing Care team for the Northeast including Tees, Durham, Sunderland, Tyneside, Gateshead and Northumberland. The CCC service undertake assessments and case management across the Northeast as outlined in the National Frameworks for Children's Continuing Care (CCC) utilising a partnership approach
The Senior Case Manager job plays a crucial role in providing specialised clinical knowledge and expertise in relation to the reviewing of ongoing care and support needs for children and young people who are subject to an element of health funding. The Senior Case Manager will collaborate with children, young people, their families and/or carers, their representatives, as well as other professionals including health, education, and social care. The Senior Case Manager will participate in and oversee regular patient reviews to ensure the least restrictive practice is implemented and to always offer specialist advice to make sure that, if necessary, clear plans for discharge are in place.
Main duties of the job
This role will be responsible for:
Following the care planning process, to meet with clients, carers and families to review care packages to ensure that individual health outcomes are recovery focussed and are being met appropriately by the care placement/package to ensure care provision is beneficial to the clients needs.
Identifying and discussing possible changes to care packages which could be highly contentious, with children, young people, families and carers as appropriate and makes recommendations to the MDT accordingly.
Being a member of the assessment and wider Multi-disciplinary Team (MDT) process with health and social care practitioners, children, young people, their families and/or carers, and/or family representative to ensure each individual has had the opportunity to voice their opinions, knowledge and experience around the individuals care needs.
Supporting and advising the allocated care co-ordinator where required to develop children, young people care and support plans to effectively meet needs.
Advising and supporting the allocated care co-ordinator, where applicable, forobtaining the informed consent of children, young people to undergo the Section 117 review, which includes implicit consent to share information with other named professionals prior to commencement of the process.
Reporting possible Safeguarding issues and serious untoward Incidents for referral onto the relevant LA for action.

To reduce the numbers of individuals who no longer have ongoing section 117 support needs by:
Identifying the numbers of individuals who require a review.
Participate in CPA and CTR reviews and provide care co-ordination guidance.
Working alongside individuals and their families and carers to offer support and advice regarding the processes.
Working alongside partner agencies and stakeholders through the process.
Ensuring that NECS policies and procedures are followed and ensuring good practice at all times.
To ensure that any review care co-ordination processes are inclusive of the individual and their families and carers and are of good quality and focused on delivering desired outcomes, by:
Supporting care co-ordinator to arrange CPA reviews in a timely manner.
Ensuring that the individual is informed of the review and is fully prepared of any potential outcomes.
Ensuring that the individual is supported as required i.e. families, carers and advocates are included within the review.
Using local processes for escalating any changes to care packages or funding streams.
