Job summary

There is an exciting opportunity for a qualified nurses with experience of working with children and young people with complex health needs, mental health and/or a learning disability to join the children's continuing care (CCC) service for the North East and North Cumbria on a part time basis. The children and young people's health assessor will be based at either Newcastle or North Cumbria and will be supported by the children's continuing care nurse specialist and the CCC Lead

The CCC team part of the All-Age Continuing Care service for the North East and North Cumbria Integrated Care Board covering fourteen Local Authorities. The CCC service undertake assessments as outlined in the National Frameworks for Children's Continuing Care utilising a partnership approach. The children and young people's health assessor role have a key role in leading the multi-disciplinary assessments, case managing and commissioning packages of care for children and young people eligible for CCC funding. The health assessor work in partnership with children, young people and their representatives, other professionals including health, education, social care and the voluntary sector. The health assessor will also be supporting the expanding all age continuing care team and ensuring the key performance indicators for the ICB are achieved. The role is the first point of contact for children/young people whilst maintaining patient confidentially at all times and each nurse has a case load to manage

Main duties of the job

This role will be responsible for:

Clinical assessments of the health needs of children and young people to determine eligibility for CCC funding.

Coordinating and completing MDT assessments and reviews in accordance with the National Framework.

Provide a high standard of holistic health assessments within the agreed timeframes.

Commissioning of the packages of care to meet the unmet health needs of children and young people eligible for CCC.

Supporting children and young people with the offer of a personal health budget to meet the health outcomes.

Case management for children and young people eligible for funding via CCC.

Record keeping as per NECS policy and procedures.

Working in partnership with children, young people, and their representatives

Building and maintain relationships with Local Authorities, providers, Foundation Trusts, and the voluntary sector

Contributing to the ICB's achievement of their statutory duties

Supporting the transition of young people to adulthood

Contributing to the ICB statutory duties for SEND

In order to be successful in this role you will need:

Excellent communication skills

Excellent organisational skills

Experience of undertaking health assessments and identifying health needs

Experience of systems including Microsoft programmes, clinical systems

Motivation to improve the outcomes for children and young people

Ensure that the health care needs of children and families eligible for Continuing Care are identified and met in accordance with the National Framework.

Support the Lead Specialist Nurse and Continuing Care Strategic Lead in the development of a range of services for children with complex health needs including palliative care services.

Undertake specialist nursing assessments for children with complex needs in relation to care packages, respite care and placements for continuing care.

Collate and undertake comprehensive and coordinated assessments for decision making for NHS Continuing Care with children and families within health and social care and care plan and commission services to meet the needs of eligible clients.

To be instrumental in the development and maintenance of effective systems, procedures and pathways to support Continuing Care and Personal Health Budgets.

To promote Personal Health Budgets (PHBs) ensuring integration into continuing care and that they are offered where requested to those clients who meet agreed criteria.

Attend the panels were arranged to present own cases and provide clinical information.

Case manages continuing care clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective.

Assist in maintaining a database for all patients assessed for continuing care and ensure high quality assurance systems are in place.

Develop strong working relationships with partner agencies including Acute Hospital Trusts, local authorities, the Independent Sector, and others.

Provide professional nursing advice to Acute Hospital Trusts, nursing homes and social services with regards to Continuing Care and Personal Health Budgets.

Work with the continuing care team, ensuring that the reviews are carried out. Following the national framework with regards to timelines of assessment and reviews. There maybe a need to refer to the continuing care manager.
