Nurse Manager

30-37.5 hours/week

£45,531 Per Annum, Pro Rata

We're looking for a Nurse with the skills and experience to lead a large team of Nurses and HCAs to deliver outstanding care to our population of over 15000 patients. Working as part of a team, you'll have the chance to shape how we do things, and to make a difference in our community here in Darlington.

You'll be a Nurse who has developed skills as a manager, as well as in patient care, with experience in General Practice, and strong knowledge of the range of services we deliver, from long term condition management, to vaccination campaigns.

Main duties of the job

The Practice Nurse Manager will be responsible for the management of the practice nursing team, consisting of nurses, HCAs, and Nurse Associates.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

The Practice Nurse Manager will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

The Practice Nurse Manager will be responsible for several clinical areas such as infection prevention and control, vaccination campaigns, chronic disease management, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

  • Registered Nurse (Nursing and Midwifery Council).
  • Post graduate diploma or degree (Chronic Disease Management).

Desirable

  • Mentor or teaching qualification.
  • Nurse prescriber.
  • Completed or working towards formal infection prevention and control training.
  • Family planning qualification.

Experience

Essential

  • Experience of working in a primary care environment.
  • Experience of leading a nursing team in a primary care environment.
  • Experience of chronic disease management.
  • Experience of working autonomously.
  • Experience of working as a practice nurse or community nurse.
  • Experience of infection prevention and control measures.

Desirable

  • Experience of managing elderly and housebound patients.
  • Experience of quality initiatives, i.e., benchmarking.
