Job summary

The Advanced Nurse Practitioner (ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The ANP provides an opportunity for patients to receive timely care and negate unnecessarily delay in receiving treatment and without the need to refer to a GP.

ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results.

They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

The post holder will be an integral part of the general practice team, working to support the requirements of the Network Contract DES across Durham West PCN.

Main duties of the job

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload & staffing levels:

Develop, implement & embed health promotion & wellbeing programmes

Manage patients presenting with a range of acute & chronic medical conditions, providing subject matter expert advice

Implement & evaluate individual specialised treatment plans for chronic disease patients

Identify, manage & support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

Request pathology services as necessary

Process and interpret pathology and other test results as required

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Maintain accurate clinical records in conjunction with extant legislation

Ensure read/SNOMED CT codes are used effectively

Prioritise health issues and intervene appropriately

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Be an extended and supplementary prescriber, adhering to extant guidance

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

Daily coffee meeting- informal opportunity to mix with colleagues to discuss matters clinical and otherwise!

Surgery hours 8.30-6, no extended access requirement (opportunity to work extended hours/Federation shifts on a voluntary basis if wished)

Skilled MDT including 4 partners, 5 salaried GPs, diabetes specialist nurse, pharmacists, pharmacy assistant, community nurse practitioners, nursing team, care coordinators and social prescribing link worker.

Qualifications

  • Registered Nurse with Nursing and Midwifery Council.
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020.
  • Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) route.
  • Qualified Independent Nurse Prescriber on the NMC register.
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
  • Meets the standards for registered ANP working at advanced level.

Desirable

  • Qualified triage nurse.
  • Minor illness qualification.
  • Teaching qualification.
  • ALS and PALS.
