Job summary
The post-holder will work to support the Service Design Manager with the effective commissioning of highly complex individualised care packages and will be required to undertake commissioning reviews to ensure that care and treatment objectives are clearly established by the clinical team and met within agreed timeframes. The post-holder will also work with the commissioning manager to implement the NHS Transforming Care Agenda. This is to support the delivery of appropriate care to meet identified needs in the least restrictive environment, to maintain the commissioning of person-centred and cost effective care packages as well as reviewing such as required. They will be an active participant in the Individual Mental Health and Disability (IMHaD) Group where applications for NHS funded care are considered and where recommendations are made to the Mental Health and Learning Disabilities Resource Allocation Panel (MH & LD RAP).
Main duties of the job
The post holder will work with commissioned NHS, social care, independent and voluntary sector providers to develop wraparound packages of support that facilitate the timely and safe transition of individuals with Mental Health needs and Learning Disabilities from acute and specialist care settings.
Operate effectively in a flexible and demanding environment and proactively engage with NHS and independent sector staff, consultants, clinical teams, and contractors working on a variety of topics. Our key partners are:

- Educated to degree or equivalent level or Nursing qualification or equivalent Occupational Health,
- Physiotherapist, Social Worker qualification with current registration
- Evidence of post registration education and training, supplemented by diploma level specialist training, experience, short courses

- A minimum of two years extensive MH/LD experience
- Excellent written and verbal communication including providing and receiving complex, sensitive or contentious information
