The post holder will be responsible for co-ordinating the process of determination of eligibility for Continuing Care within the parameters of the National Framework for Children and Young People's Continuing Care 2016 for children and young people registered within Bury/Stockport ICB.

To undertake functions, in accordance with the National Framework for Children and Young People's Continuing Care (2016), with particular responsibilities for individuals who have complex health care needs, this includes all categories of needs e.g. physical, end of life, Mental Health and Learning Disability needs.

All functions to be carried out jointly with individuals/family/carers and with other professionals from health and social care, to ensure the health needs of individuals are identified and appropriately met.

Undertake Nursing assessments &checklists.

Collating comprehensive evidence for Multi-disciplinary Team Meetings, organising and chairing Multi-disciplinary Team Meetings, completion of DST and preparation for presentation at panel.

Main duties of the job

Children's Nurse/Neonatal Nurse

Undertake comprehensive assessments, reviews and complex case management in a specialist area (Continuing Care). Evaluate and analyse information gathered during comprehensive assessments, which can often be complex and sensitive.

Provide a service to children and young people, including individuals with complex health needs, mental health difficulties and/or Learning Disabilities.

Offer Specialist knowledge across the range of work procedures and practices, underpinned by theoretical knowledge or relevant practical experience.

Undertake specialist Continuing Care and Personal Health Budget training.

Undertake self-directed learning regarding associated legislation, policies and guidance.

Completion of mandatory and job specific training; including safeguarding children and adults, Mental Capacity Act training and Deprivation of Liberty Safeguards Training.

Gain on-going experience in implementing the Continuing Care framework and providing and managing Personal Health Budgets.

Provide training sessions for health and Social Care professionals. Support individuals and carers in understanding the Continuing Care assessment process; this includes individuals who may have a cognitive impairment, which may require the use of adapted methods of communication.

The post holder will be required to assimilate highly complex, often contentious and highly sensitive information then translate this into various forms dependant on the recipient to ensure clear but accurate information is shared in a user friendly format.

The post holder will be required on a daily basis to analyse highly complex, multi-faceted information to explore different options available, using their judgment in respect of actions required and then negotiate and influence others.

PLANNING & ORGANISATION

The post holder will be responsible for management of their own caseload and arranging reviews/ co-ordinating the commissioned care package/ reviewing

The post holder will organise and coordinate MDT meetings with the professionals involved and patients/ families.

PATIENT/CLIENT CARE

Responsible for monitoring and ensuring that the standards of nursing care are meeting the expectations of the ICB, in both the hospital, respite provision and specialist placement settings in line with Ofsted and CQC standards.

POLICY & SERVICE DEVELOPMENT

The post-holder will plan and co-ordinate where required, individualised multi-disciplinary case conferences to clarify contentious issues about clinical care and plan and implement highly complex packages of care.

The post- holder will, where required to do so, actively participate in service reviews co-ordinated by other commissioners of providers who are commissioned by GM ICB- Bury/Stockport and its partnership agencies.
