Case Manager, Specialist Neurorehabilitation

Job summary

To provide a case management function for patients receiving slow stream neurorehabilitation services. This includes regularly monitoring progress against rehabilitation goals and attending multi-disciplinary team (MDT) meetings to discuss progress and support the discharge planning process. The post holder will also support NHS commissioned Level 1 and 2b services and will facilitate the assessment and costing process for patients whose needs sit outside such routinely commissioned services. The post holder will also build and maintain relationships with key individuals across NHS inpatient services, local authority, community services, independent sector (IS) providers and Continuing Healthcare (CHC) teams to ensure that a consistent and high-quality service can be provided.

Main duties of the job

The post holder will:

Provide specialist case manager in-reach support for Level 1 or 2b NHS commissioned services.

Identify patients in NHS commissioned Level 1 or 2b beds that may require slow stream rehabilitation outside of core commissioned services and facilitate the assessment and costing process for these, submitting costings and assessments for authorisation through the agreed governance routes.

Provide a robust specialist case management service for a caseload of patients in slow stream placements, commissioned either within the NHS or the Independent Sector (IS).

Attend regular MDT meetings for those on active caseload.

Ensure that data is captured as per local processes so that performance against identified key performance indicators (KPIs) identified can be monitored and reported against in line with agreed processes.

Ensure that data to support monitoring of NHS commissioned services is captured via UKROC.

Contribute to the development of improvement plans and provide regular updates of progress against these where performance against KPIs falls below the agreed standard.

Work with both internal and external colleagues to minimise the risk of delayed transfers of care (DTOC), taking appropriate action to address these by facilitating discussions to resolve issues, escalating issues encountered appropriately.

This includes, but is not limited to, GM Local Authorities, NHS neurorehabilitation inpatient services, CHC teams, community neurorehabilitation teams (CNRT) and IS providers.

Provide both line management and clinical supervision to members of the GM specialist case management team where this is required.
