Silverdale Medical Practice is looking for a 4 session Salaried GP to join our well-established forward-thinking stable team. We are a friendly, well-organised, high achieving practice within Salford, looking to replace a departing Salaried GP.

You must be enthusiastic and keen to be actively involved in all practice activities and practice development and be dedicated to providing a high standard of patient care.

12,700 patient GMS Practice.

Tier 2 sponsorship status

Pier Support

4 Partners, 4 Salaried GPs, 1 PA,3 ANP/NPs, 3 Practice Pharmacists, 3 Practice Nurses and HCAs.

Excellent Management and administrative support staff.

Main duties of the job

The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a.Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

b.Provide advanced, specialist care to patients as required in accordance with clinical based evidence, NICE and the NSF

c.Request pathology services as necessary

d.Process and interpret pathology and other test results as required

e.Maintain accurate clinical records in conjunction with extant legislation

f.Ensure read codes are used effectively

g.Maintain chronic disease registers

h.Prioritise health issues and intervene appropriately

i.Support the team in dealing with clinical emergencies

j.Recognise, assess, and refer patients presenting with mental health needs

k.Extended and supplementary prescriber, adhering to extant guidance

l.Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

m.Contribute to practice targets (QOF etc.), complying with local and regional guidance

n.Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

o.Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
