Senior Practice Based Clinical Pharmacist - Practice Based Pharmacist Team - Band 8A

With the development of Primary Care Networks (PCNs) in Bolton we are looking to recruit a pharmacist to our existing team to support our GP practices in delivering excellent patient care.

We are looking for an innovative and enthusiastic pharmacist who is committed to maximising the health outcomes of patients. This will be in a direct patient facing role supporting practices with delivery and oversight of the PCN service specifications such as structured medication reviews in addition to functions supporting day to day practice work. You will also be managing PCN pharmacy team members and working closely with the PCN clinical director to deliver for the health needs of the network population.

Job responsibilities

The post holder is an experienced pharmacist, who acts within their professional boundaries, to develop, manage and mentor a team of pharmacists and technicians. They will also actively support the PCN clinical director in delivery of the PCN goals and key pharmacy related deliverables of the national contract.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice

  • Post Graduate Diploma or Completion of CPPE Pathway
  • NMP Qualification or working towards
