We're looking for a motivated, empathetic, and forward-thinkingDietetic Assistant Practitionerto join our nationally recognisedLymphoedema Wales Team. Led by experts in the field, including our National Lead Dietitian, this is your chance to be part of a pioneering service that's shaping the future of lymphoedema care across Wales.

Main duties of the job

Under the direction of the National lead Dietitian for LWCN (and /or Registered professionals plan, deliver and evaluate group-based nutrition support and weight management programmes tailored for patients with lymphoedema

Support patient engagement and monitor progress throughout the programme.

Carry a caseload autonomously and Conduct follow-up activities for patients previously assessed for nutrition support, as delegated by the National Lead Dietitian.

Provide practical guidance and monitor patient adherence to dietary recommendations.

Undertake routine anthropometric measurements to support clinical decision-making as well as analysing and interpreting dietary information

Maintain and update the Dietetic Outcomes Spreadsheet to ensure accurate data recording and reporting.

Support the development and scheduling of regular communications, including social media content, to raise awareness of lymphoedema and promote healthy lifestyle messages.

Assist in organising and delivering events aimed at increasing the profile of lymphoedema and the role of dietetics in its management.

Contribute to the development of educational materials and resources for patients and the wider community.

Engage with wider Health Board initiatives and training programmes to promote integrated working and raise awareness of lymphoedema across services.

  • Formal qualifications equivalent to HNC/BTEC LEVEL 4
  • Nutrition Qualification Agored Cymru level 3 or equivalent
  • Willing to complete Agored Cymru Level 4 Assistant Practitioner Qualification

Desirable

  • Membership of the British Dietetic Association
  • Teaching or structured education qualification

Knowledge

Essential

  • Diverse Nutritional knowledge including Nutrition support, weight management and diabetes
  • Behavioural change or motivational interviewing
  • Knowledge of basic medical conditions and an understanding of health procedures relating to the work area.

Desirable

  • Structured weight management education and knowledge of chronic conditions e.g lymphoedema
  • General Health & Safety, Risk awareness, with the ability to relate this to individual circumstances and analyse actual risk and take appropriate actions.

Skills and Experience

  • Relevant experience of working in NHS or Social care setting
  • Experience of delivering group education
  • Good communication skills with staff and patient/clients
  • Experience of team working
  • Ability to keep accurate legible, patient/client and departmental records.
