Job summary
Provide support service to inpatient and outpatient patients based on needs, and research evidence.Participate in the multi-disciplinary team, bring youth and community perspective.Take on individual referral for teenagers and young adults experiencing difficulty in managing hospital admissions/procedures. Using evidence - based practice to asses, plan, implement and evaluate interventions.
Main duties of the job
Co-design, lead and deliver recreational programmes designs to reduce psychological impact of renal treatments and to promote adjustments, post treatmentsProvide and lead on a wide range of cognitive, recreational and social activities. To enable social and emotional development throughout the renal disease trajectory.Mentor discreet goal, under guidance of the psychological team.
