Community Triage Nurse (SPOA)

Job summary

An exciting opportunity has arisen for an enthusiastic, dynamic Band 6 District Nurse to join our District Nursing Service.

The post holder will be expected to provide professional nursing care and support to community patients , carers and families. They will also provide support to patients/carers of all ages , their carers and families by providing a holistic professional virtual nursing assessment of need and coordinating the provision of appropriate care.

The post holder will assess and triage scheduled and un-scheduled referrals into the District Nursing Service in line with the District Nursing priority levels and will use professional judgment to determine appropriate nursing care, initiating and co-ordinating input from other services as required.

They will be a motivated, reflexive practitioner and will be a key member of the District Nursing Service, working closely with the wider District Nursing Team in the provision of high quality, holistic care from the point of referral. They will possess well developed leadership and management skills, with the ability to motivate individuals and the team, whilst offering ongoing support and guidance to staff.

The post holder will occasionally be required to support clinically as per service need during times of extenuating circumstances such as; infectious outbreak, complex safeguarding, complex palliative care.

Main duties of the job

This post primarily covers an evening shift but there may also be a requirement to work days/afternoons in Single Point Of Access.

Be a V150 Nurse Prescriber

Hold or working towards the District Nursing specialist practice award.

At times of need, transport required equipment and consumables across sites in a timely manner.

Undertake comprehensive virtual assessments of patients individual care needs as devised by the nursing process.

Assist in the delivery of care that is person centred and under-pinned by effective risk management e.g. adherence to the codes which govern the Protection of Vulnerable Adults, the application of the Mental Health Act (1983) and any other relevant legislation.

Manage a caseload of patients as required, as directed by the Team Lead.

Work independently and flexibly within healthcare, education and community settings as required.

Support individuals and their families to manage their condition through education and the promotion of self[1]management.

Contribute to the overall evaluation of district nursing services

Take an active role in contributing to service development within district nursing.

Have responsibility for an allocated caseload as required, which would include the assessment of care needs, and the

development, implementation, and evaluation of programmes of care, in partnership with client and carers/families.
