Job summary
An exciting opportunity has arisen to appoint a highly motivated Health Care Support worker to join a well-established home oxygen and Non Invasive Ventilation ( NIV) service. This role will be working within the Swansea Team.
The post will be based at Singleton Hospital, but you may be required to work at other sites across the Health Board. Patients will be seen in the Out Patients Department, and also at home across Swansea Bay.
You will assist with review of patients and undertake capillary blood gas measurements, following training and home risk assessments.
You will be working alongside the MDT from primary and secondary care to assist with holistic assessment of patients and agree a plan of treatment and care with the patient at the heart of the decision making process .
The role also requires you to support patients with palliative needs, supporting patients and their families and liaising closely with community palliative care services to ensure continuity of care.
You must be organised, confident, have experience of working as a Health Care Support Worker and have excellent communication skills to enable you to liaise effectively with the multi-disciplinary team and work with a diverse and varied group of patients and carers.
Experience in Home oxygen assessment or NIV is not essential but evidence of your wider respiratory knowledge, clinical skills, experience and a willingness to learn must be demonstrable.
Main duties of the job
Assisting with Reviewing Home Oxygen and Non Invasive Ventilation patients in Out Patient Department and at home.
Performing Capillary Blood Gases and Observations of Vital Signs
Providing Nutritional Advice when Required
Supporting Patients with Palliative Care
Undertaking Home Risk Assessments for Oxygen Patients
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
