Job summary
An exciting opportunity has arisen for an enthusiastic Health Visitor to join the Child Disability Team within the Swansea Bay Health Board. Other equivalent qualifications and relevant experience will be considered for this role.
The post holder will be a Health Visitor that will offer a family centred resilience assessment approach, support and advice in a range of settings including persons own home, clinics and groups. They will be part of a dynamic team from various sectors including health, medical, local authority and the volunteer sector to provide advice, support to children and their significant others in an often vulnerable time pre and post diagnosis.
The disability team is an innovative team that seeks to promote evidence based research and practice in order to view the family holistically and promote children and their significant others to reach their full potential.
Main duties of the job
The post holder will be responsible for a caseload , which will involve assessing, planning, implementing and evaluating the care of the child with additional/complex health needs. The role will involve working with children and families at home and clinic settings. Part of the role will involve setting up and delivery of workshops on such topics as toileting, sleep, behaviour and development.
Multiagency working is an integral part of the role, therefore, good communication skills and working as part of a team is essential. The post holder will undertake home visits to children and young people and assess the care needs and implement a planned programme of care, completing referrals to other professionals and agencies, ensuring informed consent is obtained. The post holder will provide advice/support on key public health messages to children and young people/parents/carers.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
