Job summary
The post holder is responsible for the assessment, planning, implementation and evaluation of evidence based nursing care, working collaboratively and cooperatively with others to meet the needs of the patients and their families.
The aim of this service is to try and reduce admissions into hospital and expedite early discharges home. Providing acute care in the patients home.
This is a referral based service, referrals come from hospitals, GP's, GPOOH, Paramedics and so on.
You will work autonomously in patients' homes, primary care premises and hospitals to support the acute caseload holder in coordination of the patients care. You will have good communication skills and support and encourage patient/carer interventions and education.
Main duties of the job
- Under the supervision of the caseload holder, undertake an initial assessment including the taking of a comprehensive history.
- Utilise skills in medicine management to support patients and the carers in the safe use of medication.
- assist the caseload holder to plan interventions in the light of patients expressed wishes and goals of care, facilitating discussion wit the family and significant others concerning end of life issues where appropriate.
- Promote and support partnership working with other professionals, agencies, clinical services and users of the service.
- Review care and treatment plans regularly.
- Ensure all clinical records are kept in line with NMC guidelines.
- Accountable for own professional action; not directly supervised.
- Manipulation of fine tools to support patient care including equipment and medicine administration devices.
- Occasional physical support of patients including potential contact and assessment of body fluids.
- Plan own activities with caseload holder to provide robust patient care.
- Involvement in transfer/discharge arrangements to ensure the transfer of care is not delayed and is as seamless as possible.
- Build relationships across professional and organisational boundaries, breaking down barriers and smoothing the patient journey.
- Participate as a member of the multidisciplinary team through development of collaborative innovative practice
