Job summary

We are seeking a dynamic, highly motivated person with experience of working with people with acute Mental Illness. Previous experience in a similar area of work will be beneficial, with knowledge and experience of providing care in a community setting.

Swansea Assessment Home Treatment Team (AHTT) provide community care of those experiencing mental health crisis within the Swansea Locality. The team comprises of a Team manager, OT manager, and registered nurses, OT and OT Tech and Health Care Support Workers. Working as part of a multi-disciplinary team, including Medical staff, pharmacy and third sector services.

Main duties of the job

  • Excellent communication skills, a positive attitude to working with people suffering with mental illness and experience of working effectively within a multi-disciplinary environment.
  • Ability to work independently and positively with service users and the ability to develop therapeutic relationships in order to promote recovery.
  • Commitment to developing their skills.
  • Work at or from any additional location as may be determined by the health Board from time to time. You may also be required to travel within the Swansea Area as may be required for the performance of your duties.
  • Share knowledge and skills in planning, implementation, evaluation and documentation of programmes of nursing care with the team members and report any changes directly to the charge nurse.
  • Take responsibility for resources and equipment available to support nursing care.
  • Basic I.T. skills and contribute to Health Board data collection.
  • Work closely and communicate effectively with other members of the AHTT including GP's, nurses and other professionals, maintaining confidentiality at all times.
  • Establish and maintain effective communication with patients, colleagues, statutory and voluntary agencies.
  • Participate in clinical supervision, appraisals and develop a personal development plan.

Essential

  • Minimum of Qualification Credit Framework - QCF Level 3 (formerly NVQ Level 3)
  • Evidence of maintaining their personal development, demonstrates an understanding of the scope of the role in the context of the nursing team.
  • Evidence of working within specialist care setting. Knowledge and understanding of the Code of Conduct for Healthcare Support Workers in Wales.
  • Knowledge of the fundamental's aspects of care and the importance of the promotion of health and wellbeing, understanding of basic physiology, e.g. normal vital signs, fluid balance, nutritional requirements, Understands the principles of confidentiality, and principles of consent prior to undertaking care behaviour and wellbeing/mental requirements health or learning disability requirements.
  • Understands the principles of Safeguarding adults/children to keep patients within their care safe, understanding of when to seek advice and escalate to the appropriate registered professional for expert help and advice, Understanding of the Health Board's Values and Behaviour Framework.
