The overarching aim of HMP Swansea Healthcare, in partnership with SBUHB is to make a significant contribution to reducing re-offending and improving rehabilitation. Central to achieving the aim of Healthcare is to pro actively improve the health of the prison population and promoting a far greater degree of self responsibility in maintaining health status.

Main duties of the job

  • To plan and deliver high quality nursing care as a member of a multi-disciplinary team to the prisoner population who have mental health needs
  • Nurse led clinic i.e triage, vaccinations, CCM
  • To promote the overall health and well being of the prisoner population using evidence based practice
  • Be an effective member of the Prison Healthcare Service
  • Support and make a professional contribution to the development of prison healthcare services, in accordance with Prison Service and SBUHB directives

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  1. Assess the patients nursing needs, plan, implement and evaluate nursing care in order to ensure the delivery of effective patient care.
  2. Ensure the principles set out in the Fundamentals of Care are incorporated into daily practice to ensure a high quality service for the patients and clients.
  3. Maintain patients nursing records to ensure information is accurate and up to date.
  4. Ensure that patients and carers/relatives are involved in the planning and delivery of care.
  5. Ensure patient needs are met by working collaboratively with otherprofessionals and agencies, especially in relation to ongoing care needs.
  6. Ensure that effective communication is established and maintained with patients and carers/relatives, taking accurate message information on the phone or from seniors, relaying all environmental and patient information to the nurse in charge and remain polite and courteous at all times.
  7. Communicate complex and sensitive information to patients/families, including patients with special needs/learning disabilities or other barriers to communication.
  8. Assess patients suitability for discharge following medical review and instigate discharge planning arrangements.
  9. Liaise with multidisciplinary teams as appropriate to assist in complex discharge planning.
  10. Administer medication e.g. intramuscular/subcutaneous medications, etc ensuring that procedures are compliant with Health Board Policy.
  11. Carry out procedures in relation to care of patients e.g. catheterisation, removal of sutures, aseptic technique etc
  12. Ensure that the patient is safely prepared for and escorted to/frominvestigations.
  13. Recognise and respond appropriately to urgent and emergency situations. Participate in the responsibility to maintain a clean environment and ensure all members of staff follow cleaning responsibilities.
