Job summary
The Band 5 Flow Facilitator will work with the integrated multi-disciplinary team within the Neath Port Talbot Community Resource Team. The Flow Facilitator will be responsible for the supervision and management of a team of Reablement Support Workers. They will also work closely with the rapid discharge MDT to ensure safe transition of patients out of secondary care.
Main duties of the job
- To work under indirect supervision of the CRT management team to assist in the pathway process ensuring that patients are cared for in the most appropriate environment so to maximise the individual's potential.
- Assist the CRT management team in the effect planning of patients ensuring that care needs are met.
- To work with ward staff and the Trusted Assessor Therapy Team to ensure that there are no delays in maximising the utilisation care capacity.
- To manage all scheduled hospital discharges that require reablement intervention.
- To have day to day responsibilities for patient flow through the Hospital 2 Home Service.
- To perform tasks which require advanced skills in organisation and planning.
- To communicate complex information which requires negotiating, persuasive skills, tact and diplomacy.
- To provide day to day management of staff within the Hospital 2 Home rotas.
- To have awareness of Government targets and assist in the facilitation of strategies to achieve them.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
