Job summary
The Arrhythmia Clinical Nurse Specialist will work closely with the Consultant Cardiologist and Electrophysiologists developing a comprehensive support service for patients and relatives diagnosed with arrhythmias requring treatment such as, an ablation or those who require complex cardiac device implants.
The post holder is expected to apply highly specialised cardiac clinical expertise to meet the particular needs of these patients and their families. This will include undertaking clinical assessments, titration of medication, patient support and education in order to reduce hospital admission, decrease in-patient length of stay, GP consultations and reduce demand on EP consultant clinic follow ups.
Main duties of the job
Develop and co-ordinate excellent communication and working practices between hospital, community, cardiac rehabilitation and palliative care.
Develop and deliver counseling skills to convey/receive complex and very sensitive information.
Provide new/existing and/or multi-disciplinary team ICD/complex device/arrhythmia/post ablation clinics
Provide education groups or one to one consultations and reassurance to the patient group
Review patients' symptoms and medication regime. To initiate/titrate medication based on clinical assessment and need in line with Independent prescribing guidelines/scope of practice (if qualified and competent)
Establish and deliver patient support groups and/or methods of patient support (e.g. patient forums)
Liaise with consultants and cardiac nurses at referring district general hospitals, with neighboring tertiary centre and clinical physiologists to ensure a seamless service for patient group
Establish and maintain audit data to measure the effectiveness of the service
Support robust communication network with primary care, secondary and tertiary to ensure consistent, evidenced care and support for patients and relatives
Support the delivery of education programs for allied professionals including doctors & nurses.
Work in collaboration with Consultant Cardiologist/Electrophysiologists in the development and implementation new services and audit/research projects
Undertake department managerial tasks
Deputise for the senior arrhythmia nurses
