Are you looking for an opportunity to make an impact on the delivery of care to patients with Inflammatory bowel disease within Swansea University Health Board?

We are looking for a motivated and enthusiastic Nurse to join the Inflammatory Bowel Disease team. The post holder will join 3 other clinical nurse specialists in IBD to deliver care of IBD patients within the health Board. The post holder with be based in Singleton Hospital working with the Medical, nursing and existing team to deliver expert care to patients with IBD within the health Board.

You should be a registered Nurse able to provide specialist clinical support, advice and information, to the highest possible standard of evidence-based, patient-centred care for patients with Inflammatory bowel disease. You will be expected to participate in, and manage, nurse-led clinics and the direct access patient support telephone service for patients with inflammatory bowel disease with the support of the Consultant Gastroenterologists. Working hours core Monday to Friday weekend work if Service requires

Main duties of the job

The main purpose will be:1. To provide specialist clinical support, advice and information, to the highest possible standard of evidence-based, patient-centred care for patients with anaemia and inflammatory bowel disease within Morriston Hospital.2. To participate in, and manage, nurse-led clinics and the direct access patient support telephone service for patients with anaemia and inflammatory bowel disease with the support of the Consultant Gastroenterologists.3. To work as an autonomous practitioner, provide leadership and be a source of clinical expertise and professional advice, and act as a specialist resource to the patient, relatives, carers and the multidisciplinary team.4. To play a significant role in setting and maintaining standards, clinical guidelines and protocols locally and trust wide. You will be responsible for leading on nursing developments within the Gastroenterology team.5. To actively participate in departmental audit and research programmes and be actively involved in educating others and disseminating good practice.6. To work closely with the Gastroenterology Consultants and Specialist Registrars and be the Lead Clinical nurse Specialist responsible for specialised nursing management of a defined caseload of patients with anaemia and inflammatory bowel disease.7. The post holder will facilitate the management of anaemia and inflammatory bowel disease (IBD) between primary and secondary care.
