Neonatal Discharge Liaison Nurse

Job summary

The post holder will act as the lead specialist for discharge planning. Working alongside the Safeguarding CNS and daily ward manager to liaise with multi-agency partners including Maternity, Community e.g. health visitors, social services and other family support services in order to facilitate appropriate referral to ensure safe, streamlined discharge and support for vulnerable families, and the timely discharge of all babies from neonatal intensive care.

Main duties of the job

To have overarching responsibility to assess and implement the psychosocial needs of the neonate and their families from admission, during their stay in hospital and through to discharge from NICU.

The post holder will work as part of the multi-disciplinary team to ensure opportunities for Family Integrated Care (FiCare) are maximised and this core philosophy continues to be established within the neonatal unit.

The post holder will promote an innovative and progressive attitude to the continual improvement of patient and family centred care through research and evidence based practice.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Job responsibilities

  • The post holder will be a highly competent, knowledgeable and visible practitioner in NICU and will attend medical ward rounds and grand rounds with the neonatal MDT.
  • Provide support to neonatal staff to initiate the FiCare model and promote a comprehensive pathway that ensures the safe discharge of each baby from the neonatal unit.
  • Work to provide a comprehensive liaison and discharge service to clinicians, neonatal nursing staff and each family in their care.
  • Actively support the process of managing patient flow through each clinical area, ensuring the involvement of medical, nursing, therapy and administrative staff to facilitate this.
  • Work collaboratively to identify correct follow up of each neonate, liaising between families and Neonatal Consultants, therapists and the Outreach Team.
  • Ensure that every neonate and family are needs assessed for support with care planned, implemented and evaluated consistently and regularly.
  • Act as an advocate for the neonate and family ensuring appropriate information and support are provided within the boundaries of knowledge.
