Are you a level 4 Assistant Practitioner/HCSW looking for a new role that will challenge and develop you? We want confident and dynamic Healthcare Professionals to take on the role of Assistant Practitioner for the SBUHB Virtual Ward programme. We are currently recruiting into 3 permanent posts to support the Virtual Ward services in the Llwchwr, Upper Valleys and surrounding SBUHB area.
Swansea Bay University Health Board is in the process of redesigning community services as part of the patient step up/step down pathways to maximise safe and effective patient care in their own homes.
The role of the Assistant Practitioner will be to work alongside the Clinical Manager, Therapists and Medical staff linked to the Virtual Ward to provide targeted and rapid patient assessment and intervention.
The post holder will be able to help drive change in a fast-paced environment and contribute to timely solutions through engagement with relevant community partners to ensure patient treatment is optimised and harm minimised.
We are looking for applications from self-motivated, focussed individuals with proven clinical skills and a passion to improve patient care. The successful candidate will join a well-established Cluster team and will be supported to develop into this Virtual Ward Assistant Practitioner role.

Main duties of the job
SBUHB are re-designing their community step up/step down pathways to improve patient care and maximise efficiencies within the Health Board boundaries. The Virtual Ward will act as a platform for patient referrals from General Practice, Community teams, Secondary Care and Third Sector for those patients who need targeted, multi professional input in order tooptimise their clinical outcomes.
The Assistant Practitioner will be aligned to the Virtual Ward as part of the multi-professional Health and Social Care team. The Virtual Ward will bring key members of the multi professional team together to provide targeted assessment and review of patients, either virtually or faceto-face.
As an Assistant Practitioner (Level 4) the post holder is expected to independently manage their own work and case load, undertaking tasks delegated by the Virtual Ward Clinical Manager with appropriate supervision in place. Having an understanding of evidence-basedpractice and delivering care in line with current evidence.
