Job summary

We are looking for confident and dynamic Healthcare Professionals to take on the role of Senior Healthcare Support Worker for the SBUHB Virtual Ward programme. We are currently recruiting into 3 permanent posts.

The role of the Senior HCSW will be to work alongside the Clinical Managers, Assistant Practitioners and wider MDT linked to the Virtual Ward to provide targeted and responsive patient assessment and intervention. The Senior HCSW will be delegated tasks from the Clinical Manager and will hold their own caseload associated with the virtual ward register and within their scope of practice to ensure patients receive timely, prudent and safe care to help achieve stabilisation and optimisation of their healthcare needs.

The Senior HCSW will be able to work both autonomously and as part of a wider MDT, utilising effective communication skills to interact with patients, carers and colleagues at all levels. The post holder will be expected to travel across the Virtual Ward cluster footprint and deliver care within patients own homes. The post holder must be prepared to support other Virtual Wards within SBUHB when needed in order to maximise workforce and skill mix of the wider Virtual Ward team.

The areas to cover are City cluster, Bay Cluster and Upper Valley Cluster.

Main duties of the job

SBUHB is in the process of redesigning community services as part of the patient step up/step down pathways to maximise safe and effective patient care in their own homes. This will act to reduce avoidable hospital admissions and facilitate early patient discharge through the introduction of Cluster Virtual Wards. The virtual Ward will act as a platform for patient referrals from General Practice, Community teams, Secondary Care and Third Sector for those patients who need targeted, multi professional input in order to optimise their clinical outcomes.

The Senior Healthcare Assistant (Level 3) will be aligned to the Virtual Ward as part of the multi-professional Health and Social Care team. The Virtual Ward will bring key members of the multi professional team together to provide targeted assessment and review of patients, either virtually or face-to-face.

The Senior Healthcare Assistant (Level 3) has a greater degree of autonomy than the Healthcare Assistant and undertakes a broader range of health and care in accordance with policy and procedures. The post holder will work on their own initiative, undertaking delegated tasks with appropriate supervision in place from a registered practitioner/assistant practitioner within the Virtual Ward team.

The post holder contributes to assessment and assists in the development, implementation and evaluation of individualised care plans. They will supervise other staff and promote the delivery of high quality care.
