Specialised Case Manager For MH&LD Service

Job summary

Mental Health & Learning Disabilities Service Group, are seeking to recruit an experienced, self-motivated individual to work with an enthusiastic Continuing Healthcare Team (CHC) based in Bridgend to support the Mental Health & LD team with the case management of its CHC clients within their own home and in the community.

Reporting to the CHC Service Manager, you will be joining a friendly and supportive CHC team with a passion to deliver a quality service to its patients. You will have the opportunity to support some of the most vulnerable individuals in the community and follow their journey throughout the CHC process.

Main duties of the job

The successful candidate will have excellent communication and assessment skills, must be able to work autonomously but also as part of a team. The postholder will have responsibility for managing a client case load for CHC clients and ensuring the financial integrity of care provision through reviews; ensuring care needs are met, are cost effective, and are as least restrictive as possible. The post holder must be able to prioritise effectively and meet given deadlines.

You must be flexible, motivated and innovative with excellent organisational and interpersonal skills. Knowledge of the National Framework for Continuing Healthcare is desirable however not essential. You will be required to work with a wide variety of stakeholders, including patients and public, Local GPs, provider HBs & Trusts, voluntary organisations, and the Local Authority. You will be self-motivated, able to manage conflicting priorities and able to analyse complex data. You will also have demonstrable skills in developing and delivering on significant service redesign projects. Qualification and experience as a best interest assessor is desirable but not essential, as the postholder will be supported through a robust induction programme and will have the opportunity to expand their knowledge of all CHC processes.
