An exciting opportunity has arisen a Nurse Assessor to work within the Long-Term Care Team. The post holder will be part of a dynamic, friendly, and enthusiastic multidisciplinary and multiagency team.
The post holder will provide specialist assessments and reviews of health care needs for Continuing NHS Health Care individuals in liaison with Mental Health and Learning Disability Teams, Social Services, Independent Sector and Voluntary Sector.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply
Main duties of the job
A brief summary of the role includes assessing and reviewing a patients eligibility for NHS Funded Nursing Care and Continuing NHS Care. Completing Multidisciplinary Team Meetings to assess eligibility for CHC funding. Participating in Joint Monitoring of care homes alongside colleagues from the Local Authority. Undertaking investigation of DATIX and POVA incidents. Identifying and reporting areas of concern following planned care home reviews to senior managers and via Safeguarding process. Completing Best Interest meetings and Capacity Assessments. Quality assurance of Nursing Assessments. Assisting in training on the implementation of policies and guidance relating to NHS Funded Nursing Care and CHC Continuing Care, and supporting hospital staff to expedite complex discharges.

To assess and review the individuals eligibility for Continuing NHS Health Care in externally commissioned services such as care homes, independent hospitals and community setting as appropriate.
To make recommendations following Specialist Nursing Assessment.
To refer for a multidisciplinary team assessment those individuals whose physical, mental health and learning disability care needs have changed.
The post holder will be expected to undertake Best Interest Assessor training.
Through assessment, identify individuals who are at risk and act accordingly / manage any issues related to the findings.
To report any concerns related to individual care following review or assessment to the Service Manager and Band 7, Specialised Case Managers within the CHC and Long Term Care Team. Work in partnership with Social Services, CIW, Adult Protection and any relevant others as required. Assess, manage, deliver and co-ordinate the assessments of individuals utilising the effective and efficient use of resources
To attend Safeguarding Meetings as directed.
To maintain robust Specialist Nursing Assessments/Reviews and care planning records in line with relevant Health Board and Welsh Government policy.
The post holder will participate in the joint monitoring of externally commissioned services and provide comprehensive and detailed reports within the designated timeframe.
The post holder will take a lead role in the safeguarding of Adults. Will also be expected to investigate Adult Safeguarding and Datix incidents as directed by the Service Manager.
Ensure applications for Continuing NHS Healthcare are prepared and presented to the Quality Assurance meeting in a timely manner.
To promote well-being and maximise individuals and carer empowerment, where appropriate ensure individuals have access to an independent advocate.
The post holder will undertake Multi-Disciplinary Team Meetings as directed by the Service Manager and Band 7 Case Managers.
