Job summary
The role of the Cancer Tracker is to take proactive steps to ensure progression of the cancer patient journey from point of suspicion of cancer through the diagnostic pathway to diagnosis and treatment or to the point where the patient is no longer deemed suspicious of having cancer (i.e., downgraded) in accordance with Welsh Government target guidelines and requirements.
The post holder will work closely with the Cancer MDT Coordinator/Trackers and Cancer Services Management Team, liaising with Administrative and Clerical Staff to progress pathway and ensure timeliness.
To identify and highlight constraints to Cancer Services staff, including liaison with the departments involved in providing care and core services, ensuring that planned steps in the diagnostic pathway progress smoothly and in a timely manner with the overall purpose of ensuring timely diagnosis to assist with achieving Welsh Government cancer targets and positive patient outcomes and survival.
Main duties of the job
The post-holder will support the service management team to collect all relevant data throughout patients' pathway of care in order to support the achievement of the Welsh Government Tier 1 Cancer Waiting Times Target.
As part of this role he/she will liaise closely with other departments and individuals within the Health Board, expediting and escalating all possible delays which would result in a breach of the targets to the appropriate designated senior staff.
