Job summary

An exciting opportunity has arisen for an experienced and motivated nurse to join the Persistent Pain Service in SBUHB as an Advanced Clinical Nurse Specialist. Due to a current vacancy, we are looking for an exceptional individual with expert knowledge and experience of managing patients with persistent pain.

This post will involve working with a multidisciplinary team (MDT) including Consultants, Clinical Nurse Specialists, Physiotherapists, Psychologists, Occupational therapists and Health Care Support Workers. Working as part of the MDT, the successful candidate will have the opportunity to extend their knowledge and experience of managing patients with persistent pain and contribute toward supporting patients utilising the biopsychosocial model of pain management.

This role requires excellent levels of interpersonal skills and communication due to the often complex requirements of persistent pain patients. The successful applicant will also require the ability to work well both within a team and autonomously. Working within the MDT, you will be encouraged to assist with service development and improvement based on emerging patient feedback and audit. The successful applicant will be required to deliver presentations for patient education in addition to working autonomously within a clinic setting to appropriately assess and establish patient management plans.

Main duties of the job

The post holder will be a proactive MDT member of the Persistent Pain Service (PPS) ensuring that patients receive the highest standard of care and have access to comprehensive individualised support.

The role will involve supporting the lead CNS in taking responsibility for developing and leading nurses and HCSW's in the management of persistent pain within the context of Clinical Governance, directorate and national guidelines.

The successful applicant will be required to autonomously assess patients, establish biopsychosocial pain management plans including meds and monitor patients receiving pain management interventions in order to minimise and effectively manage associated side effects.

The role also requires taking responsibility for the delivery of specialist education to patient, carers and other members of the MDT in addition to being an expert point of contact for other services.

The post holder will be responsible for supporting patients in making coproductive, informed decisions by providing evidence based advice and information, empowering patients to optimise wellbeing.

The role also requires the successful applicant to support other members of the MDT in triaging referrals into the service requiring expert knowledge of PPS pathways in order to facilitate efficiency within the service and provide effective patient care.

The successful applicant will be required to liaise with services within primary and secondary care to facilitate continuity of patient care.
