Job summary

This is an exciting opportunity for a Registered Nurse with Chronic Conditions Management experience to work within Bay Health Cluster Network as an autonomous practitioner within a wider cluster multidisciplinary team.

Main duties of the job

The Chronic Conditions Nurse will be an expert generalist whose role will involve providing advanced chronic conditions management care within the community setting. The post holder will work autonomously in patients' homes and in primary care premises, to co-ordinate the care needs of those patients on their daily caseload. The aim of the CCN role is to tackle the current health inequalities amongst housebound patients and target those with limited mobility and support needs who live within the Bay Cluster boundary.

About us

The Chronic Conditions Nurse will be employed by SBUHB but funded and aligned full time to Bay Health Network Cluster to fulfil the responsibilities of this job description. Joint line management will be provided between SBUHB and Bay Health Network Cluster.
