We have an exciting opportunity for a registered Nurse to join our regional District Nursing Single Point of Access.
We have one 22.5 hour position available.The successful applicant will provide virtual professional, holistic nursing support to patients, carers and families within the community setting, delivered with compassion, dignity and respect. This post will require an individual that has a commitment to continuous professional development.
Applicants to have good IT skills with working knowledge of Microsoft Office and excellent communication skills with a flexible and adaptable approach to changing work patterns.We offer mentorship, working life balance and exciting professional development opportunities to meet your needs.
The post holder will be involved in triaging urgent and non-urgent nursing care to patients in their own homes and other community settings. They will deliver care to complex patients in the community.
Main duties of the job
Single Point of Access Staff Nurse band 5 position to cover the District Nursing Single Point of Access Service.
The staff nurse will be required to provide professional triage and support to patients, carers and families.
The member of staff will be required to function as a member of the multi-disciplinary team, assess individual patient's care needs and subsequently develop, implement and evaluate appropriate plans of care.
The role will also include the supervision and instruction of unqualified and junior members of nursing staff.
The members of staff would be required to attend training to maintain mandatory skills alongside developing new skills to meet the provision of care required.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
