Job summary
An exciting opportunity has arisen for an enthusiastic Health Visitor to join the Health Visiting Team within the Neath Port Talbot area.
The post holder will be responsible for delivering the Healthy Child Wales health visiting programme whilst implementing public health initiatives. You will be expected to work in partnership with other professionals and agencies to address health needs - demonstrating excellent communication skills whilst working both autonomously and as part of a team.
The post holder will need to be committed to developing a high degree of standards of care and actively seek and promote professional working relationships with the wider team and statutory and voluntary practitioners. The post holder will be passionate about promoting specialist knowledge and advice to families enabling them to lead healthy lifestyles, prevent illness and to give their children the best possible start in life.
In order to apply you will need to possess a Health Visiting qualification and be registered in the appropriate part of the NMC register. Travel is an essential part of this role, please demonstrate your ability to fulfil this criterion in your application.

Main duties of the job
To work with the health visiting team in NPT and wider multi agency team to deliver services to Children and young people. The post holder will deliver the Healthy Child Wales Programme to a defined caseload within a practice area, either in the home or clinic setting. This will involve assessing, planning, implementing and evaluating client care and providing advice. Plan, organise and participate/ delegate activities for children, young people and families e.g., clinics, health education groups, public health advice, screening, home visits and drop-in sessions. Undertake home visits to children and young people and participate in the assessment of care needs and implement a planned programme of care. Participate in the immunisation programmes in line with the Healthy Child WalesProgramme. This may include domiciliary immunisations. Complete and participate in developmental reviews of children and young people in accordance with the Healthy Child Wales Programme.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
