Community Mental Health Nurse (RMHN)

Job summary

To work within a multidisciplinary team and provide care and support under the Mental Health Measures 2010 for people with Complex mental health conditions. This role will also require multi professional approach and communication. Robust mental health assessments and risk management. Initial assessments while completing extended assessments to establish a treatment pathway for the individual. Communication from family and cares are essential, with collaborative working with treatment options, care and treatment plans and safety plans. The CMHT Bridgend operate 7 days a week, with one member of staff working the weekend with HTT support.

This role is fixed term/secondment for 12 months due to service needs. If you are interested in applying for it as a secondment, please obtain permission from your current line manager prior to applying.

Main duties of the job

Working to the Mental Health Act 2005, this will be updated in February 2026.

Planning care, goal setting, risk management and risk formation.

For a holistic care and risk management Communicating with patients, carers, family including muti professionals e.g. Children services, MARAC, police, GP's, third sector, LMPHSS, HTT, MASH.

Documentation/ongoing referrals are essential and there will be many CMHT, MHA etc documentation/referrals that will/maybe needed to be completed e.g. Tribunal reports, CTP, WARRN, assessment tools, daily notes and ongoing referrals.

The role does require lone working and the CMHT ensure that staff's safety in paramount and therefore have measures in place to help reduce any risk to staff. The individual will also have a responsibility to contributing to this procedure.

This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
