We're looking for a dedicated Memory Service Practitioner to provide compassionate assessment, support and guidance to clients and their carers across a wide range of multi-disciplinary services within CTMUHB.

Main duties of the job
Welsh Skills Essential: This post is advertised as Welsh skills are essential. This means Speaking & Listening at Level 3 (equivalent to CEFR B2) or above. We don't need fluency. Level 3 means basic conversations with patients about their everyday health. For more information, see 'Welsh Language Guidance' in the documents right at the bottom.
This role will work under the supervision of the Memory Service Clinical Nurse Specialist offering advice and support to clients who are experiencing suspected Dementia/Mild Cognitive Impairment, as well as their carers.
Take responsibility for a clinical caseload providing care and treatment to clients referred into the Memory Service.
Foster robust working relationships with all partners in particular Local Authority and Primary Care.
Contribute to the continuing improvement and development of dementia care services.
Promote physical and mental health wellbeing, emphasising living well with dementia.
Be an advocate for clients living with dementia and their carers, and signpost as necessary to statutory and voluntary services.
Complete assessments and follow up reviews for people within the Memory Service.

Develop robust links with colleagues in Primary Care to ensure the maintenance of the clients physical health care needs.
Work as part of a multi-disciplinary team fostering good communication with colleagues of all disciplines and the voluntary sector.
Contribute to effective communication with all parts of the mental health service, in particular the Older Adults Community Mental Health Teams (OACMHT).
Establish rapport and maintain supportive professional relationships with clients and their carers, even when there are barriers to acceptance such as lack of insight or difficulties adjusting to complex life changes; recognising the stress associated with caring, offering and promoting carers assessments as required.
Liaise with statutory and voluntary sector agencies to meet client needs.
Ensure record keeping meets the agreed standard required, utilising paper-based or IT systems as directed by the Senior Nurse and ensuring information sharing protocols are understood and appropriately explained to clients.
Present cases to the multi-disciplinary team considering the range of complex patient care and treatment options available, in line with the current evidence base and making recommendations regarding the appropriate course of action.
Assess and manage critical/unpredictable situations that arise.
Work closely with clients and carers in assessing the individual needs and ability to accept very sensitive and potentially distressing information regarding the illness and services.
Carry out day to day supervision of staff and other resources including the provision of Clinical Supervision.
Present information clearly to patients, carers and other healthcare staff.
Attend and participate when appropriate in multi-disciplinary team meetings.
Maintain accurate records of treatment and home visits providing statistical information as required.
Manage and maintain a diary of all work activities, making it available for audit of work and travel as necessary.
Be responsible for ensuring the safety and wellbeing of self and colleagues via lone working and end of day procedures.
Monitor clients physical health using appropriate equipment e.g. thermometer, sphygmomanometer and carry out clinical interventions such as venepuncture and patient observations.
