Cwm Taf Morgannwg University Health Board are redesigning the Intermediate Care Service and therefore an exciting opportunity has arisen to employ an Advanced Clinical Practitioner within our Hospital at Home service in Bridgend.
The post holder will deliver early supported hospital discharge alongside an urgent community crisis response service (providing care 8am to 9pm, 7 days a week).
The post holder will play a crucial role in providing autonomous clinical assessment, examination and management to support individuals, with a focus on promoting independence, dignity, and well-being.
The post holder will work as part of a multi-disciplinary team consisting of Consultants, Advanced Practitioners, Nurses, Physiotherapists, OTs and Health Care Support Workers. The wider Community Resource Team which includes Social Workers, SALT, Dietitians, Pharmacy and Mental Health Professionals also provide input to meet individual needs. The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing clinical oversight to be managed at home.
We will consider applicants who can demonstrate they are a good fit for the service and have relevant, transferrable skills, education and experience to be taken on in a trainee ACP role under the Annex 21 agreement.
Main duties of the job
The Advanced Clinical Practitioner (ACP) will work as part of the multi-professional team within the Intermediate Care (Hospital@Home) Service and will support the Lead ACP in the future development of the Intermediate Care (Hospital@Home) Service.
Predominately, the role is clinical and practitioners are required to see and assess people in their own homes who present with a wide array of clinical and often frailty related problems. Taking a clinical history, undertaking clinical examination and work up with our Consultants and delivering treatment and care form the core of the role.
The post holder will be responsible for providing support to the team leader in the day-to-day management of a virtual caseload of patients and ensuring that clinical practice is evidence based, safe, prudent and effective. The post holder will work autonomously within their scope of expertise utilising advanced skills in clinical assessment, examination and independent prescribing in the provision of management plans for patients.
Practitioners will also be involved in training and education initiatives including engaging with the Universities and Practitioners will be involved in the audit, research and strategic development of the service.

Annex 21
This is a training and development role under Annex 21 of the Agenda for Change Terms and Conditions.
The Advanced Clinical Practitioner will be required to undertake up to 24 month training/development programme upon commencement of this role, with an aim to attain the required advanced skills, knowledge and independent prescribing qualification.
Completion of the qualification is a requirement of the contract of employment.
During this training period, salary will be paid in accordance with Annex 21 of the Terms and Conditions as follows :
Months 1 - 12 of the training period will be paid at 70% of the top of Band 7 pay scale,Months 13-24 of the training period will be paid at 75% of the top of Band 7 pay scale,
Upon successful attainment of the qualification, even if sooner than the 24 month allocated timeframe, the post holder will automatically move to the bottom of the Band 7 pay scale (without financial detriment), as they will then meet the full KSF outline of this role and be expected to undertake all duties within this Job Description as indicated.
NOTE : There will be some circumstances where current staff apply for these posts, and their new trainee salary will be below that of their current salary. To aid both the recruitment and development of staff it is recommended that staff across NHS Wales should have continuity of pay for the duration of the training to avoid financial detriment.
