Job summary
The post holder will be working closely with colleagues in the multidisciplinary team and wider services to help children, young people, families and carers to address mental health issues and the resulting effects for them.
Main duties of the job
To provide patient centred care of a complex nature in a variety of settings.
Work with the specialist nurse to provide on-going assessment, planning, implementation and evaluation of patient care, ensuring the maintenance of clinical records. Also, to be responsible for developing plans of action, in conjunction with Specialist Nurse. Work with the Specialist Nurse in the management of a caseload, working closely with the multi-disciplinary team, patient, family and any other agencies.
At times work within the community setting, involving close family members where required, with an aim to promote positive change within the family. Attend and contribute to core groups, review meetings, child protection meetings and any other clinical forums requiring key-worker involvement and provide accurate reports for such meetings ensuring patient involvement where required.
