Job summary

An exciting opportunity has arisen to join our @Home Service team in the locality of Pontypridd as an Advanced Clinical Practitioner/Trainee Advanced Clinical Practitioner.

The @Home Service will be part of the wider Hospital @ Home service which provides a hospital admission avoidance service, responding rapidly to people in crisis because of frailty and/or managing acute medical problems at home.

This is a multi-disciplinary team of Consultants, Advanced Practitioners, Nurses, Physiotherapists, OTs and Health Care Support Workers and a Mental Health Professional.

While Professionals who meet the person specification and hold appropriate M.Sc. level education (such as an M.Sc. in Advanced Clinical Practitioner and Independent Prescribing qualifications) are desirable, we will consider applicants who are currently undertaking their advanced clinical studies and can demonstrate they are a good fit for the service and have relevant, transferrable skills, education and experience to be taken on in a trainee ACP role who will be remunerated in accordance with Annex 21 terms and conditions.

Main duties of the job

Any applicant taken on in a trainee ACP capacity will be offered the position under Annex 21 of the Terms and Conditions for Agenda for Change.

Details are contained in the attached JD/PS titled ACP Annex 21. If you are already employed in NHS Wales, there will be no financial detriment as per the Annex 21 Guidance Document attached.

The role also has substantial leadership and management aspects with Practitioners taking the lead of a team of multi-professionals, delegating and allocating work, managing an acute and clinical caseload and acting as both a gate keeper to the service as well as the point of clinical escalation.

The post holder will assist the Lead Advanced Nurse Practitioner in the clinical assessment of patients who have a complex and undetermined presentation working alongside medical and GP colleagues to maintain patients in a community setting and avoid hospital admissions and facilitate discharges.

Practitioners will also be involved in training and education initiatives including engaging with the Universities and Practitioners will be involved in the audit, research and strategic development of the service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
