Job summary
The post holder will be responsible for the leadership and development of specific Community Services: this will include a diverse range of community services and related functions; supporting the effective working across the interface of community teams, clusters and secondary care, social care and 3rd sector.
The role will oversee the new Hospital@home service and community discharge teams.
As a key member of the Primary and Community Care Group leadership team the key focus will be supporting the operational delivery with the team managers and clinical leaders, embedding the community transformation agenda and new ways of working as business as usual. Focused on safe, effective and person- centred care, you will lead the delivery of integrated community services . The post holder will take the lead in the development of efficient pathways to support patients across the wider system.
The post holder will be a person focused leader who has a proven track record of delivering high quality services, leading by example, show a commitment to team working and driven to supporting and developing staff.
Main duties of the job
Managerial responsibility for the effective operationalisation of the community transformation plans, ensuring delivery of the organisational quality, performance and financial targets in line with the Care Group and Health Boards aspirations.
Work with the Senior Management Team to ensure the strategic direction, as defined within the Health Board is effectively implemented at an operational level. for the Hospital@Home and Discharge Service.
The post holder is a member of the Senior Management Team and as such has responsibility to contribute to the re-design of services and to participate in the direct management of services and provide sound advice and guidance to the Directorate Manager and wider Leadership Team.
Work collaboratively with the other Integrated teams and professionals across the Locality and Health Board footprint to ensure consistency of approach and roll out of best practice, and seamless pathways of care.
Ensure that there is a clear accessible referral system and pathway into and within the service to relevant care providers, and to monitor its effectiveness through the appropriate performance framework.
Monitor and maintain high standards of health and care service delivery, and to provide innovative solutions to improving service quality.
Regularly monitor and review the service plans and adjust for changes in local need and in response to changes in national policy guidance.
