Job summary

The applicant will be applying for the position of support worker within the Memory Assessment Service in the Rhondda Valley.

The Support Worker will act as an expert resource providing specialist advice and support to clients and carers after a diagnosis of Dementia.

Working collaboratively across a range of multi-disciplinary services and agencies.

To become a named contact for clients and carers following assessment and diagnosis at the memory clinic.

They will also complete home visit to clients that are needing additional support and signposting to other services.

Build the clients confidence to attend social groups.

They will attend community groups to network and by a contact when required.

Main duties of the job

To support the provision of Health and Social Care to the people of RCT and Merthyr Tydfil who are living with a diagnosis of dementia in the community within the catchment area of Cwm Taf Morgannwg University Health Board. Working alongside a MDT and multi-agency team.

The post holder will regularly be required to work without direct supervision.

The post holder will be expected to assist in the organization, facilitation and running of groups within the community setting and hospital settings.

Where required to meet the need of the service the post holder may be required to work outside of their normal base/area.

Work within a Multidisciplinary Team under the direction of the CNS and Dementia advisor.

Provide support to service users on an individual and on a group basis within Community settings.

With supervision and support from CNS and Dementia advisor be responsible for and manage own workload and safety.

Participate in caseload management supervision from CNS and Dementia advisor.

Actively participate in continuing professional development through the Knowledge Skills Framework.
