In CTM, we're building our services to make a real difference -- and you could be part of it. Our @Home service is based at Dewi Sant Health Park and across RCT and Merthyr, and we are looking for a Band 6 Occupational Therapist to join us.

As an Occupational Therapist, you'll play a key role in shaping how we deliver care. You'll be assessing and supporting individuals in community settings, including their homes, and linking closely with integrated Health & Social Care teams.

Main duties of the job

- Work autonomously across community settings, undertaking lone working in service users' homes
- Manage a defined caseload, delivering specialist assessment and intermediate care
- Apply evidence-based, person-centred approaches to plan and evaluate interventions
- Maintain accurate clinical records in line with professional and local standards
- Develop your skills and expertise through a personal development plan
- Provide leadership and supervision to new staff and students
- Collaborate with a wide multidisciplinary network including hospital teams, care managers, third sector organisations, and primary care
Your impact will be seen as admission avoidance and reduction in hospital stays, while improving independence at home.
This is a full-time role, working Monday to Friday, 8am -4pm ,offering a consistent schedule while making a real impact to support your patients to live as independently as possible.

Essential
- Degree/Diploma in Occupational Therapy
- Registered member of the Health Professional Council. Evidence of commitment to CPD Post graduate training/ accreditation in fieldwork education
- Knowledge and understanding of factors which can influence discharge planning including: cognitive, physical, psychological and cultural
- Knowledge of broad range of medical, surgical and orthopaedic conditions
- Knowledge and application of principles of clinical governance and risk management Experience of service delivery in a variety of different clinical physical health settings
Desirable
- Management qualifications e.g. ILM
- Knowledge of standardized and non-standardized assessments that can be used with service user group
- Knowledge of national drivers for health provision for service user group e.g. Setting the Direction, Together for Health, Integrated Approaches, NSF
- Interest and involvement in quality, audit and service evaluation
- Ability to work flexibly and respond to unpredictable, challenging work environments
