Generic Therapy Assistant Practitioner

Job summary

We are redesigning our community services model and implementing a new Intermediate Care Team and therefore an exciting opportunity has arisen to appoint General Therapy Assistant Practitioners to help deliver an urgent community response services and will play a crucial role in developing the service, providing essential support and care to individuals, with a focus on promoting independence, dignity, and well-being.

The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or prevent admission with interventions that can be managed at home.

Main duties of the job

The post holder will work within the Cwm Taf Morgannwg University Health Board Intermediate Care Service. Supporting the registered Occupational Therapists and Physiotherapists within the Intermediate Care Service in the delivery of intervention for patients in their own homes and community.

Carry a delegated caseload of non-complex patients to deliver interventions prescribed by and under supervision of an Occupational Therapist or Physiotherapist.

To prioritise own work and carry out duties, as prescribed by the Occupational Therapists and Physiotherapists and AHP Lead. Will work independently within Occupational Therapy and Physiotherapy standard operating procedures referring to qualified Occupational Therapist and Physiotherapist where necessary.

To work as part of the Intermediate Care Service multidisciplinary team to undertake specific clinical and administrative tasks, liaise with key stakeholders such as Local Authorities and the third sector.

Support the AHP staff by assisting with all modalities of therapeutic care focused towards expediting hospital discharge and maintaining the patient within their own home and the prevention of avoidable hospital admission.

To undertake environmental assessment, deliver and fit assistive equipment in the patient's home.

Participate in service assessment, audit, and data collection to demonstrate effective outcomes of AHP intervention.

Promote independence, health, and wellbeing.
