Job summary
An exciting opportunity has arisen for a Lead Advanced Nurse practitioner for the Acute Frailty Service within the Princess of Wales Hospital.
The post holder will provide strategic clinical leadership for care of the elderly services, bringing added value, with the aim of challenging and influencing the direction of the services and working as a highly specialist practitioner in these areas. The role will ensure the maintenance of clinical excellence, and support a values based culture that creates a positive working environment. A focus on clinical policy and service development being fundamental to the role.
Main duties of the job
The post holder will provide services for patients across a broad spectrum of clinical arenas within the context of Acute Frailty/Care of Elderly Medicine. They will be responsible for a group of patients within the ambulatory frailty service, some of who will have been directly identified within the Acute Frailty Unit and Emergency department as patients who will be provided follow-up in the a ACP led Ambulatory Frailty service.
As a senior clinician and decision maker, they will directly establish and provide independent, accredited, autonomous investigations through a comprehensive physical assessment with the aim of formulating a diagnosis and formal management plan from admission to having the direct responsibility for medical discharge with no medical input.
The Lead Advanced Clinical Practioner will take an active role in developing Advanced Practice across Cwm Taf Morgannwg University Health Board (CTMUHB) and will be a part of University Health Board Advanced Practice Board.

The post holder will provide full patient care i.e. independently investigate/diagnose patients referred to the service and subsequently inform patients of diagnosis, request further investigations and liaise with multi-disciplinary members.
The post holder will be in receipt of a formal non-medical independent prescribing qualification in order to be responsible for prescribing and reviewing patients drug treatments.
The post holder will be recognized as an acknowledged expert in the management of patients with chronic disease and maintain clinical excellence in this role through the maintenance of a live portfolio.
The post holder will undertake research and audit which will directly improve and impact on patient care.
The post holder will provide supervision, teaching and development opportunities for nursing, medical and other related professionals.
