We are now hiring Community Health Care Support Workers to join our Intermediate Care Team at Cwm Taf Morgannwg (CTM). These are Band 3 jobs and are part of our new way of working in the community.
You will help people in their own homes as part of our Urgent Community Response service, which runs from 8am to 9pm, every day of the week. The jobs are based in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend.
These are permanent roles, and we have both full-time and part-time hours available. You must be able to travel throughout the day between homes across the Merthyr, RCT and Bridgend areas. As some locations may not be on a public transport route, you'll need to be flexible and able to travel in a timely manner.
This is a great chance for anyone who wants to grow their career and make a real difference. You will help people recover and stay independent after being unwell or in a crisis. You will also work closely with a team of Doctors, Nurses, Therapists, and other health professionals.
Main duties of the job
Key Responsibilities Include:o Help patients with washing, dressing, and other personal care.o Undertaking the monitoring of vital signs.o Take blood samples (venepuncture).o Follow a care plan to help people stay at home and be more independent.o Notice if someone's health is getting worse and tell a nurse or doctor.o Write down what you do and talk clearly with patients and the team.
This role is ideal for someone with experience in care work who is passionate about supporting people to regain their independence, dignity, and well-being. You will work closely with a multidisciplinary team of doctors, nurses, therapists, and allied health professionals to help patients recover and rehabilitate following illness or crisis, all while making a direct impact on their lives.

- Work alone within scope of role and have delegated caseload of patients that have been previously seen/triaged and assessed by a registered member of staff.
- Work unsupervised, following a care plan that has been implemented by a Health Care Professional.
- Undertake enabling care as directed by a registered practitioner; this will include assistance with toileting, washing, dressing and continence care, mobility, transfers and use of potential walking aids.
- Be vigilant for cues indicating safeguarding issues.
- Escalate safeguarding concerns and reports to the multi-disciplinary team in a robust, timely manner, complying with legal requirements.
- Respond by promptly escalating concerns to a registered practitioner.
- Recognise an emergency, summon assistance and act as a member of the multi-disciplinary team within parameters of competence and role.
- Use delegated therapeutic handling techniques as required by the individual and their environment in order to safely, efficiently and effectively treat the patients according to their agreed treatment plans, goals and outcomes.
- Safely encourage a prescribed recovery programme as per competency framework.
