Job summary

We have an exciting opportunity for an experienced accredited clinical coder to join our Team based in Prince Charles Hospital.

The role of an advanced clinical coder is to analyse, extract and interpret highly complex clinical information using electronic / clinical systems, handwritten medical records, clinical correspondence, discharge letters, nursing correspondence, diagnostic tests and imaging, denoting diagnosis, co-morbidity, medical and surgical procedures, medical status and relevant family history information, accurately assigning codes using the ICD10 and OPCS-4 classifications.

The information is gathered for research, aetiology studies that occur on Local, National and Internationally levels, health trends, epidemiology studies, clinical indicators and case mix planning.

Statistical coded data produced by the clinical coder is also used for clinical governance, clinical audit, tracking patient outcomes measuring treatment effectiveness, and monitoring progress of diseases.

There is a wide recognition in the National Health Service (NHS) of the importance of accurate Coded Clinical Data and the fundamental role it plays in the management of hospitals and services.

Main duties of the job

Senior Clinical Coding Analysts are expected to produce consistent, accurate, complete and timely coded clinical data in accordance with all National Clinical Coding Standards, Welsh Clinical Coding Standards, Local Clinical Coding Policies and Welsh Assembly Government deadlines.

To be self-managing in maintaining knowledge of National Clinical Coding Standards, Welsh Standards and local clinical coding policies.

1. To work autonomously without direct instruction or supervision when self-allocating work and achievement of daily coding targets.

2. Resolve complex clinical coding queries / issues where there are a range of facts or situations requiring analysis, by referring to National Clinical Coding Standards, Welsh Standards, Local Clinical Coding Policies and relevant National helpdesk query resolutions database (DELEN).

To work as an individual and as part of team, particularly when working to tight and demanding deadlines.

Be proficient in the use of AI, digital case note systems, clinical IT systems, encoders systems and interrogating electronic patient records to abstract information.

Actively participate in clinical coding quality checks in respect of internal / external clinical coding audits

Qualification / Knowledge

Essential

  • Attained the National Clinical Coding Qualification (NCCQ - Diploma level 4) and achieved Accredited Clinical Coding (ACC) status OR equivalent
  • Advanced ongoing in-depth knowledge and understanding of anatomy and physiology
  • Advanced ongoing in-depth knowledge and understanding of diseases and related health problems
  • Advanced ongoing in-depth knowledge and understanding of medical and surgical interventions and procedural terms
  • Knowledge and experience of AI, digital case note systems and the introduction of ICD11 and OPCS 5
  • Post ACC qualification, ensure compliance in 3 yearly Clinical Coding Refresher course and achieve at least 90% in the course assessment

Desirable

  • NCCQ Clinical Coding Auditor qualification
  • NCCQ Clinical Coding Trainer Qualification

Experience

Essential

  • Significant practical coding experience across a wide range of specialties of all complexities
  • Experience of interogating both paper records and electonic systems / applications to extract clinical information for the purpose of clinical coding some of which can be conflicting

Aptitude and Abilities

  • Ability to achieve 90% accuracy in internal and external audits. Explicit attention to detail and accuracy
  • Excellent communication skills, with an ability to relay complex coding information to clinical coding colleagues, operational staff and clinicians
