It is an exciting time for therapies within Cwm Taf Morgannwg. We are seeking a dynamic, motivated and innovative Occupational Therapist to join our integrated multi-professional, national award winning service.
The Occupational Therapist within the Stay Well At Home Team works closely with the Physiotherapist, Social Workers and Support Workers to provide a rapid response service to people attending the Emergency Care Department, Clinical Decisions Unit, Acute Medical Unit and Wards.
The overall aim of the service is to avoid unnecessary hospital admissions and supporting timely discharges using a discharge to assess model.
This is a 7 day week service and staff are expected to work weekends on a rotational basis - 3 x 12 hours shifts per week (with an additional 6 hoursmake up shift every 4 weeks). Hours of work 07:30 - 20:00.

You will need excellent clinical reasoning, time management, observation and communication skills and be able to demonstrate a genuine interest in achieving well being and positive health outcomes for the population we serve.
