Job summary

An exciting innovative opportunity has arisen for a new In-patient podiatry posts, offering a rare opportunity for enthusiastic, motivated podiatrists to provide high quality care, working as part of a dedicated In-patient Foot Service in the Princess of Wales Hospital.

The post holder will work collaboratively as part of the multi-disciplinary team (MDT), providing expert assessment, diagnosis and treatment/care planning for patients admitted or who have acquired acute and limb threatening foot complications.They will work as an independent expert resource, and be a highly visible advisor regarding the management of people with limb threatening conditions in the hospitals. They will provide high quality evidence based education and support for professional colleagues, including actively promoting and participation in educational seminars for health care professionals, carers, and people with long term conditions and limb threating situations. The post holder will provide comprehensive education and training to colleagues to facilitate the embedding of the inpatient foot risk assessment tool.

This post is Fixed Term/Secondment until 31st March 2027 due to funding

Main duties of the job

The purpose of the role is to work as part of a dedicated In-patient Foot Service in the DGHs. The post holder will work collaboratively as part of the multi-disciplinary team (MDT), providing expert assessment, diagnosis and treatment/care planning for patients admitted or who have acquired acute and limb threatening foot complications.

The post holder will work as an Independent expert resource and be a highly visible advisor regarding the management of people with limb threatening conditions in the hospitals. The post holder will provide comprehensive education and training to colleagues to facilitate the embedding of the inpatient foot risk assessment tool.

The post holder will be required to set the measures and collect data to highlight the Value Based HealthCare benefits and demonstrate the improved patient outcomes from the role.

The post holder will work autonomously will be a key link worker between the secondary care services and outpatient and community provision, including community podiatrists. They will be part of the Podiatry Workforce.

The post-holder will actively promote and participate in educational seminars for health care professionals, carers, and people with long term conditions and limb threating situations.

The post holder will be required to participate in the Research and Development Programme specific to this area.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job responsibilities

Coordinator Role

Undertake daily ward rounds reviewing patients admitted with limb threatening complications, collaborating and coordinating with MDT including vascular, orthopaedics, DNS, Advanced podiatrist, TV, radiologist, Microbiology and Orthotists.

Be the link between inpatient teams and outpatient care for patients with Complex foot situations to ensure appropriate follow up on discharge locally and the pathway of care between community / primary care and secondary care is as seamless as possible.

Linking in with the AHP vascular Co-ordinator to ensure established communication to enable successful repatriation of CTM patients who are referred to centralised vascular hub as part of the SEWVN arrangement and returned to back to spoke hospital.

Ensure appropriate admission of patients presenting to the hospital and enabling admission avoidance turning patients around in line with the GIRFT Inpatient Pathway, providing first line management and arranging follow up care in outpatient or community settings. Support front door services and liaise with other relevant care provisions.

Be responsible for requesting diagnostics in a timely manner, and within professional boundaries and capabilities/qualifications hold responsibility for interpreting the findings of X-rays and pathology reports to inform clinical reasoning and decision. Taking appropriate action which may include independent prescribing and making onward referral to other disciplines internal and external to the Health Board as necessary.

Evaluate outcomes achieved through the roles and responsibilities associated with the post setting outcome measures and performance targets with their clinical lead. Identify and implement inpatient diabetes foot research and audit/Quality Improvement opportunities.

Ensure concordance to the Clinical Governance Agenda and participate in the development and implementation of professional standards, local guidelines and protocols and National and Health board Wide policies. Implementation of guidelines and protocols for specific areas of Podiatric practice which may have an impact on Podiatry colleagues and other health care staff will require discussion with and support from the post holder to ensure smooth acceptance.

Act as an autonomous practitioner being professionally accountable to apply clinical reasoning to the management of a caseload with complex underlying medical situations routinely resulting in destructive foot disease and compromised Tissue Viability. This will include:-

Maintains highly developed dexterous skills, precision hand/eye co-ordination to perform operative and psychomotor tasks. This will include the ability to appreciate degree of wound tissue resistance on debridement and hence make informed decision on appropriate scalpel technique. Ensure physical ability to maintain difficult working position for the necessary extended periods of time required to carry out highly skilled assessment and debridement techniques.

Making day to day clinical decisions on complex cases without reference to line management or professional colleagues. Caseload consisting of patients presenting with Long Term Conditions, High Risk foot pathologies and complex medical clinical needs.Conduct and record neurological, vascular, orthopaedic and social assessments utilising appropriate assessment tools, to identify factors placing the individual at elevated risk of amputation/ulceration, and risk classify the patient accordingly.

Autonomously responsible for the implementation of highly specialised care programme of offloading and advanced wound care including the address of patient acceptance and compliance, foot health promotion and delivery of education. Co-produces individual care plans using motivational interviewing and coproduction skills to optimise concordance and overcome differences of opinion and understanding.

Ensures effective communication of complex, highly sensitive, contentious and emotional information to at risk/vulnerable, terminally ill, or patients undergoing pre/post lower limb amputation and carers. This requires the use of excellent interpersonal, negotiation and counselling skills. Communication of contentious information must withstand scrutiny to avoid conflict situations relaying sensitive information and where necessary using persuasive skills.

Successfully use high level interpersonal skills to liaise with ward based health care professions, Medical, Nursing and AHPs , primary care professionals, social services and the voluntary sector to ensure the delivery of a seamless service to At Risk patients.

Follow departmental Protocols, Policies, Procedures and Guidelines, but will have the discretion to make clinical decisions within professional boundaries where clinical diagnosis and clinical reasoning support decision. Guidance should be sort from peers and supervisors.

Complies with departmental policies for accurate legible record keeping & records management. This will include the use of electronic clinical information and appointment management systems.

Take ownership and responsibility for the security and maintenance of complex clinical equipment.
