Job summary

An opportunity has arisen for an enthusiastic and motivated individual to join our well established Heart Failure service

Heart failure Clinical Nurse Specialist working with the context of a multi-disciplinary team supporting providing clinical assessment and management, health promotional support, teaching patient and staff around HF as well as be involved in research projects and support the development of the heart failure service.

The post requires the successful candidate to provide clinical and health promotional management to a wide age range of patients who present with heart failure within the context of outpatient and in-reach clinical settings.

This is a fixed term post for 12 months due to funding

Main duties of the job

Clinical assessment and management of a wide age range of patients with Heart failure

Support patient/carer and staff education around heart failure

Support research projects

Support service development

Provide teaching to patients and carers to support management of heart failure condition and symptoms. Provide teaching to a multi-professional audience around heart failure presentation and management.

To support own education development - will be required to complete Heart Failure competencies and develop/maintain a live portfolio of to support enhanced level practice.

To be involved in key research projects and demonstrate use of research to support clinical management decisions
