Job summary

This is an exciting opportunity to be part of the already established Clozapine and Lithium Clinics, whilst supporting the ongoing development of Physical Health Monitoring Clinics.

Main duties of the job

The post holder will take the lead in the assessment and monitoring needs of mental health service users who require physical health monitoring as part of their care, and will be responsible for the continuing development and evaluation of physical health and wellbeing clinics. They will support Consultant Psychiatrists in the assessment needs of service users attending such Clinics.

Manage Clozaril Clinics ensuring there are enhanced communication links with ECPMS and Sysmex in order to provide point of site testing for service users.

Use enhanced communication skills, knowledge and ability in assessing the mental state, concordance, and side-effect profiling of service users attending such clinics.. Using effective communication they will support the psychiatrist/MDT to establish the efficacy of medication and with the ongoing assessment and management of needs and risks.

Job description

Job responsibilities

The post holder will take the lead in the assessment and monitoring needs of mental health service users who require physical health monitoring as part of their care, and will be responsible for the continuing development and evaluation of physical health and wellbeing clinics. They will support Consultant Psychiatrists in the assessment needs of service users attending such Clinics. Using effective communication they will support the psychiatrist/MDT to establish the efficacy of medication and with the ongoing assessment and management of needs and risks.

The post holder will be responsible for the delivery of appropriate educational packages for service users, carers and other professionals.

The post holder will need to have undertaken or is working towards being POCH 100i trained (Point of Care Analyzer). They will need to have the ability to analyse results and link up with ECPMS (Electronic Clozaril Patient Monitoring Service).

The post holder will ensure that all aspects of Clinical Governance are adhered to within the Department, and is responsible for leading the delivery of innovative evidence based therapeutic and clinical interventions for the client group. They will also provide support and clinical expertise to Community and Ward based staff.

Develop appropriate programmes of education for service users/carers and other health care professionals in relation to the benefits and side-effects of psychotropic medications, including mood stabiliser medication, antipsychotic medications, antidepressant medication and ADHD medication.

To take, monitor and analyse blood and other investigation results appropriate to the level of practice, within physical health monitoring clinics, to ensure safety of service user, using agreed escalation processes where abnormal results are found.

Maintain own work schedule on a daily basis including prioritisation to respond to crisis situations for service users and carers.

To work alone within the context of the Health Boards Lone Worker Policy e.g. during Lithium Clinic assessments, patient reviews etc

To maintain accurate nursing records, update care plans as required.
