Supporting and progressing the development of pharmaceutical services and the integrated medicine management agenda in the UHB by ensuring safe, evidence based and cost-effective primary care prescribing within a cluster group of GP practices.-
To ensure safe, evidence based and cost-effective prescribing within a locality/cluster group of GP practices.-
To support the GP/cluster and UHB primary care plans.-
To contribute to the UHB's prescribing systems and processes as part of the Prescribing Team.
To act as an Independent Prescriber or have the ability to study and qualify as an independent prescriber.
To contribute to the training of pharmacy trainees within GP practices
About the cluster:
Bridgend North cluster has seven practices serving approximately 47,600 patients in a region of ex-mining South Wales valleys. It's an area of high social deprivation with many health inequalities where 66% of the population live in the most deprived 40% of areas in Wales.
One of eight clusters within Cwm Taf Morgannwg University Health Board, Bridgend North Cluster is made up of seven main general practices, three branch surgeries and two dispensing practices.
High rates of chronic diseases in comparison to other clusters in particular COPD and CVD. High rates of smoking and obesity.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic and self-motivated pharmacist to work with the primary care team at Cwm Taf Morgannwg University Health Board (CTMUHB). The successful applicant will work in practices in the Bridgend North Cluster. The role will involve provision of medicines optimisation support and advice to the designated GP practices in the locality and supporting GP practices in cluster priorities and initiative. If you thrive on providing expert advice on medicines, addressing polypharmacy and complex clinical medication reviews closer to the patient's home environment, then this is the role for you.
You should have excellent communication skills and the ability to work effectively with other Health Care Professionals and wider primary care teams.
The pharmacy team is both ambitious and motivated, focusing our work around the best interests of our patients and continually striving to improve our services. We have a strong reputation for supporting professional development alongside the RPS advanced practice portfolio. The Medicines Management team will provide you with support to integrate into the practices and teams.
There will be opportunities for both full time and part time working. We are looking to recruit at an 8a level ideally with an IP qualification.
