To work as part of a team and also independently to deliver quality care to patients who are awaiting complex discharge planning. To support patients with their well being while in hospital. To liaise with the multidisciplinary team and work with patients and families to facilitate a smooth discharge process.

PERMANET BASE: Llynfi Ward, Maesteg Community Hospital, Bridgend
TEMPORARY RELOCATION BASE: ward 21 Princess of Wales Hospital, Bridgend
An exciting opportunity has arisen for a suitably qualified and experienced Registered Nurse to join the Community Hospital ward team. The applicant must be a Registered Nurse, be confident and competent in a wide range of clinical skills and demonstrate evidence of continuing professional development. There is a requirement for the post holder to be flexible in their working patterns to meet service need.
There are multiple demands in this exciting role due to the challenging nature of providing care for patients who require discharge planning into a variety of settings. The post-holder will however find themselves well supported by a very friendly and enthusiastic team of nurses and HCSW.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

As a registered nurse you will be expected to deliver nursing care to patients who are awaiting discharge to preferred place of home.
You will deliver personal care , medication, Iv meds and support the ward in delivering well being sessions for patients in the dayroom.
You will also be part of the MDT and attend board round to ensure that patients are discharged safely and you will be required to support the ward manager with maintaining efficient patient flow on the ward.
While patients remain in hospital we actively support the patients to maintain their well being and encourage our patients to remain active and motivated to maintain independence as much as possible.
