The cluster is fully committed to meeting the health needs of the population it serves and by working collaboratively with a 'multi-disciplinary team' approach allows us to achieve our goals.

The Primary Care Nurse will work within the cluster multi-disciplinary team, providing a domiciliary service for the co-ordination, management and monitoring of the care of the housebound patients with long-term chronic illnesses (heart disease, COPD and diabetes).

The emphasis of this role will be on service quality, through the optimisation of care where multi-morbidity exists, with the aim of maximising wellbeing, self-empowerment, quality of life and mobility.

If you are dynamic, organised, work well within the multidisciplinary team and have an interest in developing a career in Primary Care, would like to hear from you. An induction programme and development/training opportunities will be offered.
