An exciting opportunity has arisen to join the Specialist Dementia Intervention Team in Cwm Taf Morgannwg University Health Board. The team have recently commenced an exciting program of development and are looking for two Specialist Practitioners in Dementia Care Mapping. DCM qualification is not essential and training will be provided.
The DCM Practitioner will support the development of care plans and subsequent training needs through analysis of culture and culture change in our settings. They will support care staff to understand the behaviour being displayed as an unmet need and how staff can help to meet this need with them. They will provide clinical consultation and support for staff to develop their knowledge and skills in the care and treatment of patients with Dementia in the hospital setting.
The role requires a forward thinking dynamic professional (Nurse, OT, Social Worker, Physio), committed to the delivery of evidence based practice using Dementia Care Mapping and to develop services, highly specialist assessment and interventions to Older Adults with a diagnosis of Dementia.
You will need excellent communication skills, and be able to demonstrate a genuine interest in achieving wellbeing and positive health outcomes for older adults with mental health needs.
Main duties of the job
The DCM Practitioner will use Dementia Care Mapping as an observational framework that can be used for different purposes including :
- Quality Monitoring and improvement.
- Individual assessment and care planning.
- Review of key times of the day.
- Staff development and training needs analysis.
The post holders will use the Dementia Care Mapping Model to provide support to the Older adult inpatient areas across CTM whilst supporting the existing DCM mappers with data collection, DCM cycles, supervision etc.
The post holders will be responsible for monitoring how action plans are implemented using robust quality assurance and outcome measures and the collection of appropriate data to evidence change.
The successful applicant will be have the opportunity to enhance service delivery and quality improvement and to develop advanced clinical skills.
They will be expected to provide clinical supervision to junior staff members and students and deliver formal teaching and training, as appropriate.
